Union County Hospital Home Health is an extension of hospital services and physician care. Home Health is provided to patients in their homes. Care may include an array of therapeutic and support services to give the patient maximum independence. Home Health serves the counties of Union, Jackson, Johnson, Alexander, and Pulaski.

Union County Hospital Home Health develops a plan of care with the patient's physician. A registered nurse visits each patient in his or her home. All nurses, therapists, and other staff are in close communication with the patient's physician and will notify him or her of any changes in the patient's condition or needs. Union County Hospital Home Health offers nursing services, physical therapy, speech therapy, occupational therapy, patient and family education, personal hygiene, and social services. Union County Home Health can make arrangements for special equipment, such as hospital beds and wheelchairs and housekeeping and transportation referrals.

Although a physician referral is required to initiate Home Health Care services, the following people can help make the necessary arrangements and provide further information:
